Please help us encourage the success of your child(ren) by keeping their records current and up to date.
Existing and future clients alike can access and download some state required forms here. Many of the forms are available on this website and will need to either be completed and brought to your child’s intake meeting or updated before pre-registration.
This document is to be used by parents whose child is missing required health care information in their file. We are a state licensed facility and must meet all regulations mandated by Indiana State Board of Health. Download
To be considered for admission to a Grand Tots’ program in both early education, before and after school care, and summer care. families must submit an application. Download
Use this form to update your emergency contacts as soon as there is a change in the information. Download
Medication Authorization
To protect the health and welfare of children and school staff members, Indiana laws require that school personnel observe certain safeguards in administering prescription medication to pupils. If we are to administer prescription medicine to your child, the following form must be completed by the parent/guardian and the child’s medical provider (when necessary), authorizing the administration of medication to the child while at the center. Please note that prescription medication must be labeled with: the child’s first and last name, the date the prescription was filled, the name and contact information of the prescribing health professional, the expiration date, dosage amount, length of time to give the medication, and instructions for administration and storage. To be considered for admission to our school-aged program as well as summer care, families must submit an application. Download
This form grants permission for specific individuals, such as family members or close friends, to pick up the child from daycare in the parents’ absence. Download
Use this form to update your child’s information such as change in contact numbers, address, etc. Download
If there is a change in your child’s health status or a new drug is introduced, please use this form to update the Grand Tots staff. Up-to-date health information is essential for us to provide the best care possible.  Download

How to return a form in person:

  1. Download and complete the form you need below. The form can be completed by printing out the PDF, and then filling out the requested information with a blue or black ink pen.
  2. Give the completed form along with copies of any additional documentation that may be required to a Grand Tots staff member.

  1. Download and complete the form you need below. The form can be completed by printing out the PDF, and then filling out the requested information with a blue or black ink pen.
  2. Next, scan (preferred) or take a clear photo of the completed form. A blurry or hard-to-read photo will not be accepted and you will be asked to bring the form to the main site.
  3. Finally, email the form along with copies of any additional documentation that may be required to grandtotsdaycare@gmail.com.
