Existing and future clients alike can access and download some state required forms here. Many of the forms are available on this website and will need to either be completed and brought to your child’s intake meeting or updated before pre-registration.

Enrollment Application - Early Education

To be considered for admission to a Grand Tots’ program in both early education, before and after school care, and summer care. families must submit an application. Download here

Change of Emergency Numbers

Medication Authorization

To be completed by parent/guardian and child’s medical provider (when necessary), authorizing the administration of medication to child while at the center. Download here

Make a Payment

You can make a payment comfortably from wherever you have internet access—24 hours a day, 7 days a week. Please note: It will be helpful to have your billing statement in front of you, as it contains necessary account information. We use WooCommerce and PayPal for secure transactions.
